What is the Assisted Living Waiver Program?

Eligibility Criteria for an ALW Participant

Age 21 or older;
Enrolled in SSI/SSP/SSD or receive monthly income;
Have full-scope Medi-Cal eligibility with zero share of cost;
Open to residing in assisted living as an alternative to a Nursing Facility;
Require care equivalent to Medi-Cal-funded Nursing Facility residents;
Capable of living safely in assisted living or subsidized housing;
Willing to live in an assisted living setting in counties offering ALW services: Alameda, Contra Costa, Fresno, Kern, Los Angeles, Orange, Riverside, Sacramento, San Bernardino, San Diego, San Francisco, San Joaquin, San Mateo, Santa Clara, and Sonoma.

FAQs

How do I apply for the Assisted Living Waiver?

Individuals who meet the ALW eligibility requirements, need to contact an approved Care Coordination Agency (CCA) which will assist individuals/families with the application process. Please see link above for the current list of Care Coordination Agencies and make sure to work with a CCA that operates in San Diego County.

I'm interested in Stellar Care for my ALW loved one - can I set up a Tour?

All approved/pending status ALW participants families/Power of Attorneys can have your Care Coordination Agency contact us to schedule a tour.

Our experienced staff provides a structured environment where residents with Alzheimer’s disease and other dementias can thrive with set routines.

Club Stellar

Residents who are early on in the progression of their dementia or Alzheimer’s disease may be eligible for our Club Stellar program and special rate.

Dining

Our staff prepares nutritious meals three times a day, featuring in-season fruits and vegetables, homemade soups, and diabetic options.

Activities

The Stellar Care activities staff curates tailored activities, field trips, and programs for all residents at various levels in the progression of their dementia.
