Assisted Living Waiver Program

Individuals who meet the ALW eligibility requirements, need to contact an approved Care Coordination Agency (CCA) which will assist individuals/families with the application process. Please see link above for the current list of Care Coordination Agencies and make sure to work with a CCA that operates in San Diego County.

To qualify for the Assisted Living Waiver Program, an individual must be age 21 or older, have full-scope Medi-Cal eligibility with zero share of cost, and be willing to live in an assisted living setting in a participating county like San Diego. Additionally, they must require care equivalent to that of a Medi-Cal-funded nursing facility but be capable of living safely in an assisted living environment.

Individuals who meet the eligibility requirements must contact an approved Care Coordination Agency (CCA) to assist with the application process. It is important to work with a CCA that specifically operates in San Diego County to successfully enroll in the Assisted Living Waiver Program.
