How to Move Into Stellar Care

Checklist of Items needed

1) Assessment

Stellar Care staff will do an Assessment to determine which Care Service Levels will be required to reside at Stellar Care and if the potential Resident is appropriate for our community

Items Needed Prior to Assessment:

602 Physicians Report with a Cleared TB Test within the last 6 months filled out and signed – attached
Covid Vaccination Card
Medication list – please have them add any over the counter medication to this list (aspirin, etc.) –
Family/Healthcare Agent allows our team to speak to current Caregivers and/or Facility where resident is at

2) Plan of Care/Acceptance of New Resident

After the Assessment, Stellar Care can provide a Plan of Care which will detail the Care Services we believe would be required for the potential Resident to reside safely in Stellar Care. If all parties are in agreement, we can move forward with Admissions

3) Completion of Stellar Care Admissions Packet & other items described below

Stellar Care can provide a digital or hard copy of our Admissions Packet that needs to be completed/signed/initialed by the Healthcare Power of Attorney and Financial Power of Attorney for the Resident. Stellar Care will need to countersign several of pages and we like to set up a meeting prior to move in to go over this paperwork and discuss our protocols, etc.

Items needed prior to move-in

Personal:

Copy of Photo ID of Resident
Copy of Resident’s Medicare Card. Replacement cards can be ordered: https://www.medicare.gov/basics/get-started-with-medicare/using-medicare/your-medicare-card
Copy of Resident’s Medical Insurance Card
Copy of Resident’s Advance Care Directive or a Trust that names Power of Attorney for health and finance (the complete sections of the Trust for health & finance). If you don’t have an Advance Care Directive – there is one in the Admissions packet.

From Resident’s Doctor we need:

602 Physicians Report with a Cleared TB Test within the last 6 months filled out and signed
POLST filled out by and signed front and back by Doctor – in packet
Covid Vaccination Card

Covid Test within 72 hours of move-in required
